EBA&M believes that a strong partnership with the provider community is essential to providing members with the best possible experience as they navigate the healthcare system.

Our IVR system allows you to verify a patient’s plan eligibility, obtain benefit plan information and check claims status any time of the day. You can access this easy to use system by dialing (800) 249-8440 and following the prompts.

If you prefer to use your computer to obtain the above mentioned information, then our EBA&M Online web portal is the system for you. You can click on the link to the right to either register as a new user or login if you are already registered. Once logged in, you have all plan eligibility, benefit plan information and claim status available at your fingertips.

Our provider service center is available from 6:00 to 5:00 PDT Monday through Friday if you need to discuss the information you’ve received from our IVR or web portal.
